ANNUAL EMPLOYEE EVALUATION

   
Annual evaluations occur for all college personnel.  The following forms are used in the evaluation process:
   
Unit/Group Form
   
MSEA Support Services PER 119 – State of Maine
  Performance Management Form
  (Available from Human Resources)
   
MSEA Supervisory Services MSEA Supervisory Services
  Performance Management Form
  (Available from Human Resources)
   
MEA Administrative Services Performance Evaluation Form
  (Available from Human Resources)
   
Confidentials Confidential Employee Performance Appraisal
  (located in President's Office)
   

Within 45 days of the date of the employee's annual evaluation date (based on date of hire or end of probation), the Human Resources Department will forward the following to the employee's supervisor/division manager:

1.  Appropriate form from above  
2.  Copy of the employee's job description  
3.  Personnel Action Form (generated from HR)
(Note:  #2 and #3 are excluded from Confidentials.)
   
PROCEDURE:  
   
1.  Supervisor reviews and completes all paper work with division manager.

2.  Division manager reviews evaluation with president when necessary. Upon agreement, supervisor will perform personal evaluation with employee and obtain all required signatures.

3.  Ten (10) days prior to anniversary date, the appropriate evaluation forms with required signatures and the Personnel Action Form would be forwarded to the Human Resources Department.

   

Note:  If upon review of the employee's job description, it is determined that a change in classification is necessary, all paper work for this change MUST be completed prior to the evaluation deadline.  (Also see the Faculty and Staff Handbook, No. 3.25, Annual Job Description Review.)
