All employees who
spend at least sixty-six and two-thirds percent (66 2/3%) of their time
operating a Video Display Terminal (VDT), including alternate work time,
shall be entitled to be examined annually by an eye doctor at the
expense of the College. All
employees receiving eye examinations must provide the College with a
medical release form. Employees
shall be given a VDT Operator Eye Exam Form (sample attached) to be
completed by the eye doctor and their immediate supervisor.
This form, along with a completed Internal Purchase Requisition,
must be turned in to the Business Office. Employees covered by the MCCS
health care plan are encouraged to utilize the option of bi-annual eye
exams which will be covered by Anthem Blue Cross and Blue Shield.
