New Student VA Benefits Checklist
Chapter 1607

1. ____   Complete an Application for Educational Benefits (22-1990) or Request for Change of Program of Program or Place of Training (22-1995) (whichever is applicable)

2. ____   Submit application and your DD-214 or NOBE (whichever is applicable) to:

                        VA Regional Office
                        P.O. Box 4616
                        Buffalo, NY 14240-4616

 Annotate "Chapter 1607" prominently on all correspondence and documents

3. ____   After enrolling in classes, complete a Request for Certification Form and submit to the Financial Aid Office located in Student Affairs. This form needs to be completed every semester in order for you to receive your educational benefits.

If you have used your educational benefits at another institute of higher learning, you must complete form 22-1995 and mail it to Buffalo at the above address.
