New Student VA Benefits Checklist
Chapter 30 (Active Duty) and Chapter 1606 (Selected Reserves)

 

 Complete an Application for Education Benefits (VA Form 22-1990).

 

 Submit your application and a copy of your DD-214 or NOBE (Notice of Basic Eligibility) (whichever is applicable) to:

Buffalo, NY  14240-4616

 

 After enrolling in classes, complete a Request for Certification Form and submit to the Financial Aid office.  This form needs to be completed every semester in order for you to receive your educational benefits.

 

 Submit Department of Defense form to report kicker benefits (if applicable).

 

 

 


Note:  If you have used your education benefits at another institute of higher learning, you must complete form VA Form 22-1995 and mail it to the above address.  Please be sure to scroll to the bottom of the page for all of the potentate forms.
