New Student VA Benefits Checklist
Chapter 30 (Active Duty) and Chapter
1606 (Selected Reserves)
Complete an Application for Education Benefits (VA Form 22-1990).
Submit your application and a copy of your DD-214 or NOBE (Notice of Basic Eligibility) (whichever is applicable) to:

After enrolling in classes, complete a Request for Certification Form and submit to the Financial Aid office. This form needs to be completed every semester in order for you to receive your educational benefits.
Submit Department of Defense form to report kicker benefits (if applicable).
Note: If you have used your education benefits at another institute of
higher learning, you must complete form
VA Form 22-1995 and mail it to the above address. Please be sure to scroll
to the bottom of the page for all of the potentate forms.
